What to expect at your first appointment

 

During your first appointment, we will collaboratively identify your needs and goals for treatment. We will develop a treatment plan that will guide future sessions. Prior to this appointment, you’ll be asked to complete and return a Patient Information Form.   


Online Therapy

 

Therapy takes place virtually on a HIPAA-secure video platform.

Fees and Payment

 

The hourly rate for individual therapy rate is $195 (per 45/ 50-minute session); payment is made at the time of service. 


Insurance

 

Should you wish to file with your insurance company for out-of-network benefits, you can receive a billing summary and statement. We do not directly bill third parties for reimbursement.

Additionally the fee may be eligible for payment by your FSA or HSA.


Cancellation Policy

 

If you become unable to keep an appointment, please contact Dr. Amsellem prior to 24 hours before the scheduled appointment. Appointments that have not been canceled are subject to the regular session fee. 


Communications

 

Electronic messages are reserved for scheduling purposes. Confidential information will not be communicated over email without your expressed consent, as it is not a secure form of communication. Email and voicemail messages are monitored regularly. 


Emergencies

 

In the event of an emergency, please dial 911, call a crisis support line or service, or proceed to an emergency room.
